Monthly Puppy Report

Please fill it out at the end of each month and email your puppy report to Heidi. Guide Dogs tracks each puppy's progress. Please label the puppy report as follows:

Puppyname.PuppyID.month.year

Example:  Bindi.123A.01.07

Each raiser submits a monthly report on their puppy's progress and challenges to help Guide Dogs with their later training.

Some information reported is:

  • Medications puppy is on
  • Vaccinations puppy had this month
  • Any vet trips puupy had this month
  • How does puppy do with the following commands: Sit, Stay, Down, Come on Leash, Come off Leash, Wait, Okay, Stand, Let's Go, That's Enough, Go to Bed
  • What outings did the puppy go on this month?
  • What has puppy accomplished this month.
  • What concerns do you have about your puppy?
